1.3 आवेदन पत्र के साथ स्वप्रमाणित समस्त दस्तावेज कार्यालय मुख्य चिकित्सा एवं स्वास्थ्य अधिकारी, जिला – मुंगेली (छ०ग०) जिला अस्पताल के पास, रामगढ़, जिला मुंगेली (छ. ग.) के पते पर रजिस्टर्ड डाक/स्पीड पोस्ट द्वारा दिनॉक23.02.2026 को समय शाम 5.00 बजे तक आवेदन पत्र प्रेषित कर सकते हैं। इसके अलावा अन्य किसी माध्यम से आवेदन स्वीकार नहीं किया जावेगा।
4.1 संविदा भर्ती के लिए न्यूनतम आयु 18 वर्ष एवं अधिकतम 70 वर्ष चिकित्सकीय पद हेतु तथा 64 वर्ष प्रबंधकीय पद हेतु होगी। आयु की गणना दिनांक 01 जनवरी 2026 से की जावेगी। इस आयु सीमा में सभी प्रकार की छूट सम्मिलित है।
