- EWS certificate must be valid and issued by Tahasildar/Revenue Divisional Officer in prescribed form.
- PWD candidates must possess disability certificate issued by a duly constituted and authorized Medical Board of State or Central Government Hospitals/Institutions.

Other Important Information
- Contract Duration: Full Time Super Specialist/Specialist: One year contract extendable up to three years on satisfactory performance or till regular incumbent joins, whichever is earlier. Part Time: One year contract or till regular incumbent joins, whichever is earlier. Senior Resident: One year contract or till regular incumbent joins, whichever is earlier.
- Working Hours (Full Time): 09:00 AM to 04:00 PM on weekdays; 09:00 AM to 01:00 PM on Saturdays. Emergency calls to be attended as required.
- Part Time Working Hours: 4 hours per day x 4 days per week (16 hours per week).
- Leave (Full Time Super Specialist/Specialist): 4 days per month on pro-rata basis; can be encashed up to maximum 30 days. Half-day leave treated as one full day. No leave admissible for Part Time.
- Leave (Senior Resident): One leave credited per completed working month; accumulated unavailed leaves lapse upon completion of one year tenure.
- Professional Indemnity Policy: Selected Specialist/Super Specialist must have a professional indemnity policy covering the contractual period. Minimum sum assured: (1) Rs. 30 lakh for Anaesthesiologists, Cardiac Surgeons, Neuro Surgeons, Plastic Surgeons etc.; (2) Rs. 20 lakhs for General Surgeons, Gynecologists, Obstetricians etc.; (3) Rs. 10 lakhs for Physicians, Dentists, Radiologists, Pathologists etc. Copy of insurance and premium receipt must be submitted within 7 days from date of engagement.
- Security Deposit: Selected candidates must submit a security deposit of Rs. 15,000/- as DD in favour of ESIC Account No. 2 payable at Kollam at the time of joining (refundable after successful completion of contract and production of no-due certificate).
- Agreement: Selected candidates must enter into an agreement with Medical Superintendent, ESIC Model & Super Specialty Hospital, Asramam, Kollam on non-judicial stamp paper of Rs. 200/-.
- Termination: Appointing authority reserves the right to terminate service at any time before tenure by giving one month notice or immediately in case of disciplinary action. Engaged person must give one month prior notice before resigning or pay equivalent of one month’s remuneration in lieu of notice.
- Private Practice: Not allowed except for Part Time Super Specialists/Part Time Specialists.
- TA/DA: No TA/DA admissible for interview.
- Part Time Restriction: Part time engagement of one contractual employee in two ESIC hospitals at the same time is not permissible.
- NOC: Candidates working in Central/State Government institutions must obtain NOC from concerned authority.
- DNB Training: The hospital is accredited for conducting in-house DNB training courses in some specialities.
- Vacancies: Indicated vacancies may change as per actual requirement on the date of interview.
- PWD: PWD candidates will be selected against suitable/notified posts as per government rules. PWD/Female applicants are exempted from application fee.
